PLEASE READ: 1.) ONCE YOU BOOK YOUR APPOINTMENT AND PAYMENT IS SUCCESSFULLY MADE YOU WILL BE SENT INTAKE PAPERWORK. 2.) PLEASE SIGN AND RETURN ALL INTAKE PAPERWORK A MINIMUM OF 24 HOURS BEFORE YOUR INITIAL SCHEDULED SESSION. 3.) ONCE RECEIVED, YOUR APPOINTMENT WILL NOT BE RESCHEDULED. IF NOT RECEIVED WITHIN 24 HOURS YOU WILL BE CALLED TO RESCHEDULE YOUR INITIAL APPOINTMENT. 4.) YOUR SECOND SESSION WILL BE SCHEDULED IN YOUR SESSION WITH YOUR COACH. To reschedule YOUR APPOINTMENT, YOU MUST CALL within 24 hours of scheduled appointment. There is only 1 reschedule allowed per appointment within a 30-day period. If appointments are cancelled within1 hour of scheduled appointments, or if you are a no show for your scheduled session, you will be billed a $15 fee late/no show fee. All payments are final.
